NSA 3/23/23 HBS: Hospital Discharge on Bipap

Studies using screening questionnaires have found that 40 percent of inpatients ≥50 years old and 66 to 87 percent of inpatients with a body mass index (BMI) ≥30 kg/m2 score as high-risk for OSA. Emerging data support earlier intervention with PAP therapy in patients admitted to the hospital who are found to haveevidence of OHS or COPD with hypercapnea. Hospital discharge with PAP may reduce mortality in a select group of patients following acute-on-chronic hypercapnic respiratory failure in patients with or suspected of having OHS(Obesity Hypoventilation Syndrome).

Physicians would like to know the situations in which CPAP or BIPAP is indicated on discharge and situations where testing or evaluations are needed to be completed before prescribing. 

Desired Practice: 

1. Impact of untreated sleep apnea, COPD with CO2 retention and OSA in hospitalized patients with focus on patients with chronic respiratory failure

2. Positive airway pressure therapies in hospitalized patients, indications and management

3. Post-discharge care plan for patients in whom CPAP or BIPAP is indicated on discharge and situations where testing or evaluations are needed to be completed before prescribing

Learning Objectives

1. Understand the impact of untreated sleep apnea in hospitalized patients with focus on post surgery, stroke, MI and acute on chronic respiratory failure, Understand the indications and appropriate prescription of Positive airway pressure therapies in hospitalized patients

2. Understanding when it is appropriate to discharge patients with new CPAP/BIPAP devices

3. Understand clinical situations where patients should be undergo further evaluation for respiratory devices.
