NSA 4/16/20 HOSPW: Life Care Planning & COVID 19
A meaningful discussion about Life Care Planning (LCP) would help members plan for unexpected events, to stimulate members' thoughts about their preferences in how they want to be cared for when they are not able to make decisions for themselves.
This course will explain the current Life Care Planning program, its’ resources, and of course as part of First Steps population, encourage physicians to set an example and do their own planning.
Target Audience
Adult Family Medicine physicians, Health Education and Palliative Care providers leading this initiative.
Learning Objectives
1. Initiate meaningful discussion about LCP with members.
2. Refer patients over 55 to First Steps Online by referring them to kp.org/lifecareplan or Health Education via e-consult or access to a notary to help them understand and complete an Advance Health Care Directive (AD).
3. Recognize how to support involved caregivers who request patient health information.
4. Start their own planning: Complete an Advance Directives.
