EDUCATIONAL OBJECTIVES:

As a result of attending this presentation, participants will be better able to:

(1) develop overall hospitalist role and goals; (2) improve interactions with adult and family medicine physicians and other healthcare providers; (3) work to overcome challenges such as hand-offs / continuity of care; (4) provide support for quality measures, including those addressing readmissions for chronic illnesses; (5) identify cultural/linguistic issues which may present barriers and/or opportunities in the care experience.
