Session Objectives:

At the conclusion of this event, patients will be able to: 

  1. Initiate both a beta-blocker AND an ACE-inhibitor (or ARB) in patients presenting with heart failure with reduced ejection
    fraction (LVEF<40%).
  2. Increase appropriate referrals of patients presenting with HFrEF (heart failure with reduced ejection fraction) to the Heart
    Failure Clinic.
  3. Increase POLST, Advanced Care Directives, and NEXT step referrals in patients referred to the Heart Failure Clinic.
