Session Objectives:
At the conclusion of this event, patients will be able to:
- Initiate both a beta-blocker AND an ACE-inhibitor (or ARB) in patients presenting with heart failure with reduced ejection
fraction (LVEF<40%). - Increase appropriate referrals of patients presenting with HFrEF (heart failure with reduced ejection fraction) to the Heart
Failure Clinic. - Increase POLST, Advanced Care Directives, and NEXT step referrals in patients referred to the Heart Failure Clinic.
